A minority or children with idiopathic or immune thrombocytopenic purpura (ITP) have the rerractory chronic rorm with bleeding problems (stage III or IV). The aim or this article is to provide an update on the immunopathogenesis and management or children with bleeding and severe refractory lIP. The man~lgement of such patients. according to a st~lging guideline. is described. I{ecent ciinic~11 and I~lbor~ltory observ~ltions document the disturbed immune responses th~lt occur on v~lrious levels in chronic lIP. New therapeutic options ~'re directed tow~lrds illiluencing these immullopathllgenic mechanisms.
